HSE's investigation found that the work had not been adequately planned and the risk not adequately assessed. In addition, no instruction had been given to the digger operator, and there was no segregation of pedestrians and plant in this area of the site. Read this press release for more details on the incident. Our website has guidance on working with excavators. Other recent prosecution cases include: Make sure you submit a RIDDOR report for a workplace accident or illness where necessary.
